So how do clinicians look for it? A few different methods exist, each with its own strengths and limits.
The research gold standard: the euglycemic clamp
The most precise method is called the hyperinsulinemic-euglycemic clamp. It's considered the "gold standard," but it's complex, time-consuming and expensive, so it's used almost exclusively in research settings. During the procedure, insulin and glucose are delivered through a continuous drip into the bloodstream while blood sugar is held steady, so researchers can measure exactly how well the cells are responding to insulin's signal. Because it's so involved, you won't meet it at a routine check-up โ it's mainly a tool for studying how insulin resistance works.
In everyday healthcare: fasting insulin and indexes
In a normal healthcare setting, one starting point is a fasting blood sample โ drawn in the morning before any food or drink. Fasting blood panels are routine, but insulin usually isn't included in the standard set, so it often has to be requested separately.
On its own, fasting insulin rarely tells the whole story. Instead, doctors may use an index such as HOMA or QUICKI, which look at the balance between fasting glucose and fasting insulin. The idea is simple: if insulin is running high relative to glucose, the body may be producing extra insulin to compensate โ a sign of insulin resistance. There are now also some home tests that measure insulin, glucose, or both and report an index score.
The catch is that these indexes often aren't sensitive enough to pick up insulin resistance in its earliest stages. Early on, fasting insulin can still look completely normal. That's because, at the start, the body only needs to ramp up insulin briefly, right after a meal. It's mainly the carbohydrates in food that prompt the pancreas to release insulin. Only later, as things progress, does the pancreas start producing more insulin around the clock โ and that's the point where fasting-based indexes become more useful.
There's another wrinkle: the threshold values for these indexes seem to shift depending on sex, age and ethnic background, so it's hard to draw firm conclusions from a single number. Home tests add a further question mark, since the sample has to be shipped to a lab, and that journey can affect its quality.
Often the most useful in clinic: the glucose tolerance test
At the moment, one of the more reliable options in everyday healthcare is the oral glucose tolerance test. It's the same test used to confirm pre-diabetes or diabetes โ but when insulin resistance is suspected, insulin is measured alongside glucose. You're given a standardized glucose drink, and blood samples are taken before and then typically at 30, 60, 90 and 120 minutes afterward, so the team can watch how glucose and insulin move over time.
The honest bottom line
Even here, there are no universally agreed cut-offs that say "this is insulin resistance." The diagnosis comes down to a clinician's judgment โ often a specialized endocrinologist โ and usually involves first ruling out other conditions that can cause similar symptoms. Tests like a blood lipid profile, thyroid hormone levels, and electrolyte and uric acid levels can help support the picture.
