The conditions in that cluster are familiar ones: higher blood pressure, higher blood sugar, extra body fat carried around the waist, and unhealthy cholesterol or triglyceride levels. On their own, each is worth keeping an eye on. Together, they tend to amplify one another.


How metabolic syndrome is defined

Internationally, the most widely used definition says a person has metabolic syndrome when they meet three or moreof the following five measurements:


  • Waist circumference above roughly 102 cm (40 inches) in men, or 88 cm (35 inches) in women

  • Triglycerides of 1.7 mmol/L (150 mg/dL) or higher

  • HDL ("good") cholesterol below 1.0 mmol/L (40 mg/dL) in men, or below 1.3 mmol/L (50 mg/dL) in women

  • Blood pressure of 130/85 mmHg or higher

  • Fasting blood sugar of 5.6 mmol/L (100 mg/dL) or higher


The exact cut-offs vary a little from country to country โ€” the waist measurement in particular is adjusted for different populations โ€” but the overall picture is the same. By this definition, metabolic syndrome is common: studies estimate that somewhere around a quarter to a third of adults in Western countries meet the criteria.


Having just one of these on its own doesn't mean you have metabolic syndrome. It does mean it's worth paying attention, because the more of them that stack up, the higher the risk of complications like type 2 diabetes and heart disease.


Where insulin resistance comes in

Metabolic syndrome is sometimes called insulin resistance syndrome โ€” and that name is telling. Insulin resistance is almost always one of the disturbances sitting underneath it.


Here's the gentle version of what's happening. Insulin is the hormone that signals your cells to take in glucose (blood sugar) from the bloodstream and use it for energy. With insulin resistance, the insulin is still there, but your cells respond less readily to its signal, so glucose has a harder time getting in. It isn't that anything is broken โ€” the cells simply aren't reacting the way they normally would.


You can have insulin resistance without showing any of the other signs of metabolic syndrome. But often it travels as part of a wider pattern. Many people with insulin resistance also have higher blood pressure or unhealthy cholesterol and triglyceride levels โ€” and in some cases, the insulin resistance itself is the piece that hasn't been spotted yet. So if some of the other signs are present, there's a good chance insulin resistance is in the mix too, even when blood sugar still reads as normal.


The encouraging part

Because these conditions share so much common ground, they also share much of the same path forward. The causes and the steps that help โ€” for metabolic syndrome, for insulin resistance, for type 2 diabetes โ€” line up closely with one another. And in all of them, everyday lifestyle plays a real and meaningful role.


That's the hopeful thread running through all of this. These are patterns that can shift. You don't have to change everything at once. Small, steady changes, one step at a time, genuinely add up โ€” and you don't have to figure it out alone.
